Our approach

Better health starts with what's on the plate.

Anchor Community Health delivers medically tailored meals and grocery support to eligible New York Medicaid members — because nutrition is one of the most powerful levers in healthcare, and the hardest one to use on an empty schedule.

The plan, in plain terms.

You qualify, we enroll you, meals arrive. If something changes — a new diagnosis, a tough month, a recipe that didn't land — we adjust. No long forms, no surprise bills, no questions about whether you "deserve" the help.

  1. Food as medicine

Every meal is built by a registered dietitian around the specific conditions our members live with — diabetes, heart disease, kidney care, pregnancy, cancer recovery, and more.

  2. No friction

We handle eligibility, enrollment, scheduling, and shipping. Members tell us what they need; we make it show up.

  3. Real ingredients

Fresh produce, lean proteins, whole grains. No mystery powders, no chalky shakes — food that tastes like food.

  4. Care that follows up

A Community Navigator stays in touch, adjusts the plan as health changes, and connects members to broader supports when life requires it.
